WIAA Physical Exam Permits
*Student athletes must have a WIAA Preparticipation Physical Evaluation form on file with an exam date on or after April 1, 2011 for the 2012-13 school year (located under "Forms").
*If last year's WIAA Preparticipation Physical date of exam on file is current (after April 1, 2011), the Alternate Year form is required (also located under "Forms").
Athletic Participation Paperwork:
High School Athletes: All items below are required for participation and must be turned in/submitted to the Athletics Department before the student will be able to particpate. All Forms available on the right hand column.
1. WIAA Preparticipation Physical Evaluation Form (4 pages).
Alternate Year Form available if PPE on file from last year: Date of Exam must be on or after 4/1/2011 to be valid for 2012-13 school year.
2. WIAA Concussion Law Agreement (i-Form; must be submitted for each sport played)
**WIAA Questions and Contact Information form must be printed, completed and turned in to the Athletics Department
3. Code of Conduct (i-Form; submit once per school year)
4. WIAA Eligibility Bulletin (i-Form; submit once per school year)
5. $120 Participation Fee (paid via the webstore in Infinite Campus)
