Red Cards/Blue Cards

High School Athletes:  All items are requirements for participation and must be turned in before you will be given your Red Card for the first day of practice. 

  1. Turn in your WIAA Physical Evaluation (form located in right hand column  to Ms. Trost in the Athletics Office)  
  2. The Athletic Code (i-form)
  3. WIAA Eligibility Bulletin (i-form)
  4. $120 participation fee via the Webstore in Infinite Campus. 
  5. Wisconsin Concussion Law Agreement (i-form) 

Middle School Athletes:   All items are requirements for participation and must be turned in before you will be given your Blue Card for the first day of practice.

PLEASE NOTE: ALL OUTSTANDING FEES AND CURRENT SCHOOL FEES MUST BE PAID BEFORE STUDENT IS ALLOWED TO PARTICIPATE.

Parents:  The Blue/Red Card is considered the student's "ticket to participation".  Students are expected to pick up the Blue/Red Card from the Athletic Office to take to their coach.  The Blue/Red Card indicates the student has handed in all requirements and that he/she has been cleared for participation. 

Rain outs:  In an effort to become more efficient in notifying our parents and fans of game cancellations, we would like to inform you of a way to be notified as soon as any of our sporting events are canceled, postponed, and/or rescheduled. Please log on to www.woodlandconference.org.
