Program/Services Description:
Homebound instruction will be provided to eligible students attending school within New Berlin School District. A request for homebound instruction is submitted in writing to the building principal. Additional information may be requested to substantiate the condition preventing the student from attending school for 20 consecutive school days. Homebound services will be provided for a maximum of five (5) hours per week unless the student is eligible for special education services. In such circumstances, the amount of time required for instruction will be decided by the student’s IEP team in consultation with the building administrator. Homebound instruction will be provided to a school-aged parent anytime during her pregnancy provided her physical condition prevents her from attending school. A student may receive homebound instruction following the birth of her child if the student is unable, for physical reasons resulting from the birth, to attend school.
Role of Position:
As part of the instructional team, the school nurse will review all medical information relative to a request for homebound instruction. Instruction will be provided by a licensed teacher. The homebound teacher will coordinate the homebound program with the student’s classroom teacher(s). In the event the student returns to the classroom after a period of recovery, the homebound teacher will continue to collaborate with the classroom teacher(s) until the student is current with assignments and other classroom expectations.
